Job Description:

A company is looking for a Health Engagement Coach.
Key Responsibilities:

Establish health improvement plans and personalized goals for members
Support customers in achieving health-related goals and reducing health risks
Communicate results of health reviews to relevant parties and ensure compliance with regulations

Required Qualifications, Training, and Education:

Associate's degree required
Valid, current LPN/LVN license issued by the state of residence
Ability to complete assigned work in a timely and accurate manner
Ability to work independently
Experience in customer service and remote work is a plus
