Job Description:

A company is looking for a Behavioral Health Care Management Clinician (remote in Oregon).
Key Responsibilities

Document and present clinical assessments and case formulations
Develop individualized plans of care and coordinate with various providers
Facilitate communication between members and their support systems

Required Qualifications, Training, and Education

5 years of experience in mental health and knowledge of DSM criteria
Experience with programs such as EASA, IIBHT, or coordinating care levels
Master's degree in social work, psychology, sociology, or a related field
Licensed Clinical Social Worker or equivalent Oregon licensed healthcare profession
Bilingual in Spanish preferred
